Anatomical and topographical relationships determine a close association between the nasal cavity, paranasal sinuses, and lacrimal drainage system. Pathological processes in the nasal cavity due to its immediate vicinity of the area of the lacrimal pathways may also spread to the latter. Objective — to improve surgical treatment for dacryocystitis in patients with sinonasal diseases. Patients and methods. The investigation enrolled 56 patients who were divided into 2 groups. Group 1 included 22 patients with chronic dacryocystitis who had been treated at an ophthalmology clinic, without attracting an ENT doctor and had undergone transcanicular laser dacryocystorhinostomy; Group 2 consisted of 34 patients with chronic dacryocystitis and sinonasal comorbidity who had undergone endonasal endoscopic dacryocystorhinostomy. Results. In the early postoperative period, as from day 2, Group 1 patients reported disappearance of discharge from the lacrimal puncta and a reduction in lacrimation. On days 2—3, the patients had better nasal breathing, no tearing, and systemic improvement. Normal mucociliary clearance indicators were restored in the patients of both groups within 2—6 months. Conclusion. The complex approach to treating an abnormality of the nasolacrimal canal, which provides simultaneous correction of the intranasal structures, by eliminating nasal obstruction and pathological changes in the paranasal sinuses, can enhance the efficiency of surgical treatment for chronic dacryocystitis in patients with rhinologic comorbidity.
