Screening and preventive counseling (SPC) in the primary health care (PHC) facilities is considered to be one of the main measures aimed at decreasing the level of alcohol consumption. Objective — to develop organizational and functional technology for SPC and to test the latter in the PHC facilities. Material and methods. An organizational experiment was carried out in three subjects of the Russian Federation (Astrakhan Region, Vologda Region, and Tomsk Region) to adapt SPC technology in PHC during 12 months. After the first stage of screening, positive patients were referred to the second stage. According to the results, three patients groups were formed. Group 1 patients needed no targeted counseling and received healthy lifestyle advice. Group 2 patients were provided with preventive counseling that was aimed at reducing or avoiding the harmful use of alcohol. Group 3 patients were offered advice by a narcologist. A telephone survey was conducted on patients who received consultations. Results. A total of 908 (5.9%) patients were identified at the first stage of screening. They were allocated as follows: 616 (67.8%) patients (less than 7 scores); 277 (30.5%) (8—19 scores); and 15 (1.7%) (more than 20 scores). 45 of all the patients consulted were interviewed. Of these, 55.6% adhered to recommendations to reduce alcohol consumption; 40% did not. Conclusion. A SPC algorithm was elaborated during medical examinations, which was tested in case of health care facilities in three regions. There were pronounced territorial differences in the results of universal and targeted screening. Testing of this organizational and functional technology showed its feasibility in terms of practical public health.
