
Новая парадигма лютеиновой поддержки в протоколах вспомогательных репродуктивных технологий: системный прогестерон как ключ к успешной имплантации и живорождению
Резюме
Прогестерон является ключевым системным гормоном в программах вспомогательных репродуктивных технологий, а наибольшее значение контроль его уровня имеет в циклах криопереноса на фоне циклической гормональной терапии, когда дефицит может негативно влиять на исходы беременности. В таких циклах мониторинг уровня прогестерона в сыворотке крови позволяет персонализировать поддержку, а применение водорастворимого прогестерона для подкожного введения обеспечивает надежную системную коррекцию при его субоптимальных значениях или непереносимости вагинальных форм. При сохраненном овуляторном цикле предпочтение может быть отдано естественным и модифицированным естественным протоколам, тогда как выбор упреждающей системной поддержки, сроков снижения дозы прогестерона при беременности и тактики у пациенток группы риска следует осуществлять на основе индивидуального подхода. Эти аспекты должны стать предметом дальнейшего изучения. Цель данного обзора: определить оптимальные подходы к лютеиновой поддержке и персонализации применения прогестерона в программах криопереноса эмбрионов в зависимости от клинической ситуации и уровня прогестерона в сыворотке крови.
