Septic abortion is a current public health problem due to high maternal mortality and subsequent reproductive disorders. Purpose — evaluate clinical efficacy of cytoflavin in the complex treatment of septic miscarriage and risks of developing distant reproductive disorders in chosen cohort of patients. A prospective randomized study of 68 patients with septic miscarriage during 8—20 weeks of pregnancy, who received hospital treatment was carried out on the base of the gynecological department of the Research Institute of Emergency Care named after I.I. Janelidze in 2014—2016. Depending on the treatment regimen, patients were divided into 2 groups: in the first group (n=34) patients received cytoflavin in the infusion therapy scheme — 10 ml diluted in 200 ml of saline or 5% glucose solution, twice per day with of 12±2 hours interval for 5 days (10 administrations of the drug) from the first day of hospital stay. Patients of the second group (n=34) received treatment according to the standard regimen. Laboratory examination, in addition to generally accepted, included (1 and 7 day of observation) determination of pro-inflammatory cytokine production level, lipid peroxidation (LPO) antioxidant system (AOS) activity in dynamics. All patients underwent a comprehensive assessment of reproductive function not less than a year after the end of treatment on an outpatient basis. Results. Inclusion of cytoflavin into the infusion therapy led to a significant increase in the activity of the antioxidant system (SOD from 227.2 to 384.4 act/min, catalase from 4.09 to 5.61 μmol/l) and a decrease in the content of diene conjugates (from 3.93 to 2.24 µmol/l), inflammatory markers (CRP — from 12.4 to 3.4 mg/l) and pro-inflammatory cytokine (IL-6 — from 10.22 to 1.98 pg/ml). The inclusion of the drug in the treatment regimen reduced the incidence of thin endometrial syndrome (5.9% vs 23.5%, χ2 =4.18; p<0.05), a decrease premature ovarian depletion (2.9% vs 20.6%, χ2= 5.1; p<0.05). Conclusion. Cytoflavin inclusion in infusion therapy contributes to the activation of the body’s antioxidant defense, reducing the intensity of oxidative processes and inflammatory reaction, which subsequently reduces the incidence of pathology associated with infertility. The revealed effect of the drug, along with good tolerance, allows to recommend its inclusion in the treatment regimens of patients with this pathology.
