Metastases of endometrial cancer in the brain are rare and not well describe. Most often, the progression of cancer of the uterine body is manifested by metastatic lesions of the lungs, bones, and lymph nodes, which is revealed as part of plane observing. Metastasis to the brain usually presents with unexpected sudden neurological symptoms, and subsequent neuroimaging reveals a cerebral lesion. Treatment of secondary lesions is critical to improve survival and quality of life. Complex local treatment (microsurgical removal of the tumor followed by adjuvant radiotherapy) combined with a personalized approach to drug therapy appears to be the best treatment approach to date. A clinical case demonstrates of management of a patient with advanced endometrial cancer, who, since March 2015, due to the progression of the underlying disease in the form of a metastatic brain lesion, underwent complex treatment, including neurosurgical interventions for metastatic foci, a session of radiosurgical treatment postoperative zone in frontal area, and 2 consecutive lines of drug treatment. Against a consistent clinical approach was achieved a stabilization of the disease within 4 years.
