The results of laparoscopic interventions in the treatment of complicated forms of gastric cancer at the Clinic of Faculty Surgery named after N.N. Burdenko are presented. To evaluate the safety and feasibility of laparoscopic interventions for complicated gastric cancer.

MATERIALS AND METHODS

The study included 88 patients (mean age 64.2±12 years), 42 were operated laparoscopically, 46 in the traditional way. Men accounted for 66%. The average body mass index is 28.6 kg/m² (19—40 kg/m²). The distribution by stages: IIIA — 28.6%, IIIB — 23.8%, IIIC — 21.4%, IV — 26.2%. The operations were performed according to vital indications taking into consideration the developed complications: cardiac stenosis (14) and pylorus stenosis (23), and hemorrhage from the tumor (51).

RESULTS

The investigators performed 28 gastrectomies with abdominal esophageal resections, 39 Billroth-I subtotal resections, 11 Billroth-II subtotal resections, and 10 proximal resections. The average duration of surgery was longer in the study group, while intraoperative blood loss was significantly lower in the open surgery group. The average number of lymph nodes removed during laparoscopic D2 lymph node dissection was 24±5.

CONCLUSION

Endovideo surgery is technically feasible, and it is possible to remove the tumor within healthy tissues (with the known advantages of the laparoscopic surgery technique and an acceptable level of complications).
