Рецессивный дистрофический буллезный эпидермолиз (РДБЭ) — один из основных типов буллезного эпидермолиза, редкое тяжелое генетически обусловленное заболевание, характеризуемое системным хроническим воспалением, наличием на коже пузырей, эрозий и обширных хронических ран, склонностью к развитию плоскоклеточной карциномы. Вызывается это заболевание патогенным вариантом гена COL7A1, кодирующим синтез коллагена VII типа (C7) — основного компонента якорных фибрилл, которые поддерживают сцепление между эпидермисом и подлежащей дермой. Заболевание носит изнурительный характер. Методы лечения в основном включают поддерживающие меры, поэтому разработка методов лечения, воздействующих непосредственно на причину заболевания, весьма актуальна. Статья посвящена возможности применения одной из наиболее перспективных стратегий лечения РДБЭ — редактирования генов у пациентов с РБДЭ с помощью специфических нуклеаз — TALEN (Transcription activator-like effector nucleases) и CRISPR/Cas (Clustered Regularly Interspaced Short Palindromic Repeats/Crispr-Associated protein), которое позволяет практически необратимо восстановить синтез C7. Метод находится на стадии доклинических исследований. Представлены результаты доклинических исследований, сравнительный анализ применяемых методик, а также подходы, которые, возможно, помогут в ближайшем будущем вывести данный метод генной терапии РДБЭ в клиническую практику.
