Fungal nail infections (onychomycosis, nail mycosis) occur in a half of all cases of nail diseases. The major pathogens causing onychomycosis are dermatomycetes (dermatophytes) and, less often, non-dermatophytes. Intricate culturing of micromycetes often leads to overdiagnosis or underdiagnosis of non-dermatophytic nail infections. Delayed identification of the etiological factor entails inefficient treatment of onychomycosis caused by non- dermatophytes. Treatment of onychomycosis caused by Fusarium spp. is of particular difficulty. This pathogen has a low sensitivity to most systemic antifungals used for treatment of onychomycosis (terbinafine, fluconazole, itraconazole). A case of successful treatment of foot onychomycosis caused by Fusarium spp. in a 51-year-old female is reported. The diagnosis was confirmed by culturing. Treatment was provided with allowance for the isolated pathogen sensitivity to antifungals. The use of pulse therapy with itraconazole (Itrazole) at the dose of 200 mg BID for 7 days during the 1st, 5th, and 9th weeks, instrumental pedicure, and amorolfine hydrochloride led to full recovery 5.5 months after treatment onset.
