OBJECTIVE

To develop a stratification scale for evaluating the mortality risk in patients with acute limb ischemia caused by embolism.

MATERIAL AND METHODS

We analyzed more than 3500 individuals with aortic and peripheral arterial embolisms. The study group comprised 954 patients, the control group — 2556 patients. Statistical analysis was performed using the Statistica 12 software. We analyzed between-group differences using the Pearson’s chi-square test and odds ratio. The scale was created using multivariate logistic regression analysis. We applied ROC analysis to assess the model’s quality and perform cross-validation. ROC curves were compared using the De Long test.

RESULTS

The STADHIS scale was created for evaluating the mortality risk in patients with aortic and peripheral arterial embolisms. Age over 80 years old, persistent arterial hypotension upon admission, acute myocardial infarction, diabetes mellitus, duration of limb ischemia >12 hours, heart failure NYHA class III-IV and decompensated or irreversible acute limb ischemia were significant predictors of fatal outcomes. Prognosis was very accurate (AUC>0.8). Validity of the scale in the independent sample (p<0.001) confirmed its potential applicability for clinical evaluation of patients with acute limb ischemia due to embolic arterial obstruction.

CONCLUSION

The STADHIS scale is significant and simple. The main application is objective assessment of patients with acute limb ischemia due to embolic arterial obstruction that may be of critical importance for optimizing treatment strategies.
