Abstract:

Aim — to analyze surgical factors associated with physiological circulation recovery and affecting internal carotid artery (ICA) restenosis in long-term after carotid endarterectomy (CEE). Material and methods. There were 1,035 patients who underwent CEE in 2011—2016. ICA restenosis occurred in 71 patients. In order to determine mechanisms of ICA restenosis all patients were divided into 3 groups depending on technique of CEE: 1 — conventional CEE with patch modeling (n=22, 31%); 2 — conventional CEE without patch modeling (n=38, 53.5%); 3 — eversion CEE (n=11, 15.5%). Results. ICA restenosis occurred in 13±5.6 months after surgery on the average. Redo CEE was performed in 3 cases, stenting — in 16 patients (intraoperative TIA developed in 2 patients). Redo restenosis was absent in long-term period in this group of patients. Conclusion. CEE with patch modeling prevents restenosis due to preserved physiological circulation.
