Abstract:

Aim - to assess immediate results of «Hemiarch» procedure. Material and methods. The study included 25 patients with thoracic aortic pathology who underwent «Hemiarch» procedure 20 (80%) patients and «retrograde Hemiarch» - 5 (20%) cases. The operation was performed under cardiopulmonary bypass (CPB), circulatory arrest (CA) with moderate hypothermia (25-28 °C) and unilateral antegrade cerebral perfusion via innominate artery. Results. Duration of CPB, aortic cross-clumping and CA were 200.5±62.4, 109.7±49 and 23.7±4.4 minutes respectively. Perfusion of right hemisphere was not accompanied by deficiency of blood supply of left hemisphere. Intraoperative cerebral oxygen saturation was 63-73%. In 1 (4%) case bilateral perfusion was applied, 1 (4%) patient required resternotomy due to bleeding. Transient neurological deficite was observed in 1 (4%) case. There were no stroke and coma. Hospital and 30-day mortality were also absent. Conclusion. Reconstruction of thoracic aortic aneurysm using «Hemiarch» procedure under right-sided unilateral antegrade perfusion is justified surgical strategy.
