Abstract:

It was analyzed the results of antegrade brain perfusion in aortic arch surgery in 143 patients. An effectiveness of different modes of brain perfusion, impact of "complete" protection technique of internal organs on the frequency of complications were evaluated in the study. It was not observed statistically significant differences in the results of mono- and bihemispheral perfusion. Both methods are equally effective. Bihemispheral perfusion is suitable in cases of unlocked circle of Willis and presence of significant stenosis in right common carotid and subclavian arteries. It was shown that monohemispheral perfusion is effectively and fairly in all other cases. It was detected that "complete" protection of brain and internal organs leads to significant reduction of multiple organ failure and death frequency.
