Abstract:

Direct and long-term results (from 6 months to 2 years) of endovascular closure of patent ductus arteriosus (PDA) were studied proceeding from current state of technique development. 112 transcatheter occlusions of PDA were carried out during 2009-2011. Mean age of the patients amounted 9,2±3,4 years (from 1 to 62 years), body mass - 31,1±6,5 kg (от 7,4 kg до 70 kg). Sizes of arterial ducts varied from 1 mm to 11 mm and amounted in average 3,7±1,1 mm according to angiography data. According to Kritchenko classification PDA type A was observed in 87 (77.6%) cases, type B - in 9 (8.2%) cases, type C - in 5 (4.5%) cases, type D - in 7 (6.2%) cases, type E - in 4 (3.5%) cases. In 97 (85%) patients installation of 1 spiral was enough for achievement of optimal result. Patients with PD diameter from 1.2 to 4.5 mm formed this group. 4 patients (3.5%) with PDA diameter from 4.5 to 6 mm required installation of 2 or more spirals. Embolization of PDA using Amplatzer ductus occluder system was carried out in 5 (4.5%) patients. Direct efficacy of intervention was observed in 109 (97.3%) cases. Residual shunt was evaluated as negligible in 2.7% of cases. In moderate-long-term period residual shunt was not observed in any patient. It is drawn a conclusion that extent of endovascular PDA occlusion at present exceeds amount of open operations including ligation and clipping. Efficacy of the method amounts 97.3% in short terms and 100% in long terms after intervention.
