Abstract:

The review is devoted to pathogenesis, diagnosis and treatment of pelvic venous pain (PVP) in patients with pelvic varicose veins (PVV). Pelvic pain etiology is unclear. Available hypotheses cannot fully explain pathogenesis of PVP in patients with PVV. Incidence of PVP is different in patients with similar morphological changes in the veins. Some authors emphasize the role of vasoactive neuropeptides (substance P, calcitonin gene associated peptide, endothelin) in development of PVP. Today, there are no specialized scales for assessing the severity of PVV. Visual analogue scale for pain assessment (VAS) and Von Korff questionnaire are convenient for assessing pain syndrome. However, these measures are failed to assess severity of all clinical manifestations of PVV. The issues of pathogenesis, diagnosis and treatment of PVP are far from being solved, despite a significant number of reports devoted to PVP and PVV. A more detailed research of neurobiological aspects of pelvic pain following PVV will contribute to improvement of treatment outcomes.
