
Причины неэффективности эндоваскулярного лечения тазовых венозных заболеваний
Резюме
Эмболизацию овариальных вен (ЭОВ) и стентирование подвздошных вен широко используют в лечении тазовых венозных заболеваний (ТВЗ). Вместе с тем клиническая эффективность (купирование симптомов заболевания) эндоваскулярного лечения ТВЗ варьирует в значительных пределах. В ряде случаев эндоваскулярные вмешательства не оказывают должного положительного эффекта, а иногда сопровождаются усугублением клинической симптоматики. В статье описано несколько клинических наблюдений, которые демонстрируют неудачи эндоваскулярных вмешательств, обусловленные ошибочным выбором тактики и метода лечения пациенток с ТВЗ. Представленные случаи демонстрируют ряд очевидных причин неэффективности эндоваскулярного лечения ТВЗ: необоснованное выполнение эмболизации тазовых вен, нарушение этапности выполнения эндоваскулярных вмешательств на подвздошных и тазовых венах, выполнение спиральной окклюзии овариальных вен с развитием протрузий спиралей. Представленные клинические примеры и данные литературы указывают на необходимость мультидисциплинарного подхода к обследованию и лечению пациенток с ТВЗ, рационального и дифференцированного использования эндоваскулярных лечебных методов у пациенток с сочетанием различных форм ТВЗ.
