Abstract:

OBJECTIVE

Analysis of own experience of surgical treatment of patients with benign prostatic hyperplasia (BPH) of large size and old age or with severe concomitant pathology in whom conservative therapy was ineffective, obstructive type of urination or cystostomy persisted and surgical treatment in the proper volume was impossible because of the aggravated somatic condition.

MATERIALS AND METHODS

59 patients with BPH underwent surgery in the volume of bipolar minimally invasive transurethral resection of the prostate with low-dose spinal anesthesia on the background of long-term dutasteride therapy.

RESULT

The average duration of the surgery was 9 minutes, the lavage system was used 14.0+1.5 hours, the volume of removed tissue was 6—12 grams, the urethral catheter was removed within 24 hours in most of patients. The number of complications according to the classification of surgical complications by Clavien—Dindo in the early postoperative period was 24.8% and was mainly represented by complications of I and II degrees.

CONCLUSION

This method of combined treatment of BPH made it possible to improve the quality of life of patients, restore natural urination, significantly reducing the risks of surgery.
