The purpose. Development of optimal laparoscopic approach to the posterior segments of the right lobe of the liver. Material and methods. Since 2015, at the University Surgical Clinic OSMU on the basis of the Pirogov City Clinical Hospital, Orenburg, we had operated by laparoscopic approach 60 patients with for benign liver pathology (hemangiomas, hepatoadenomas, calcified and live echinococcal cysts, non-parasitic cysts). Of these, with localization of the pathology in SVII—VIII, there were 17 patients. Results. A non-standard approach for laparoscopic interventions on the VII and VIII segments of the liver is presented. Surgery was performed in the patient’s position on the left side at an angle of 90° to the surface of the operating table, which after dissection the ligamentous apparatus of the right lobe of the liver led to its medial displacement and getting into the plane of the operative action of the posterior lower surface of the liver. By this technique, laparoscopic interventions were performed in 17 patients with pathology localized in the posterior segments of the right lobe of the liver. Conclusion. The method allows for the necessary visualization and accessibility for free manipulation of laparoscopic instruments in the zone VII and VIII of the liver segments and, as a result, to increase the efficiency and safety of surgical intervention.
