Extranodal non-Hodgkin lymphomas most often develop in the organs of the gastrointestinal tract. They can affect any organ from the oropharynx to the rectum. Systemic antitumor therapy is the main treatment. The need for surgery procedure arises in patients with complicated forms. One of the possible complications is gastrointestinal organs wall perforation. A rare clinical case of a complicated course of gastric non-Hodgkin lymphoma with perforation of the wall and the formation of a spleen abscess was demonstrated. Taking into account the localization of the tumor, a sparing operation (proximal resection of the stomach) was performed in a minimally traumatic way for the patient (laparoscopically assisted), which allowed to accelerate the patient’s recovery in the early postoperative period. The use of intestinal insertion at the reconstructive stage (double-tract) made it possible to achieve good functional treatment results.
