Epiphrenic diverticulum is an uncommon disease of the distal part of the esophagus. We report the case of a 67-year-old man with a symptomatic esophageal epiphrenic diverticulum, achalasia and a hiatal hernia. Surgery was indicated by the patient’s symptoms and the associated achalasia. Preoperative study included barium swallow, chest computed tomography with oral contrast and upper gastrointestinal endoscopy. A laparoscopic transhiatal diverticulectomy associated with a Heller esophageal myotomy and a Dor’s fundoplication was performed. The overall operative time was 200 minutes. No postoperative complications occurred. Chest computed tomography with oral contrast performed on the first postoperative day did not completely exclude leakage from the staple line and availability of pneumonia. It required antibiotics using despite the absence of sings of systemic inflammatory response syndrome and delayed discharge. However, the following investigation did not confirm these findings. The postoperative hospital stay was 6 days. The laparoscopic approach to epiphrenic diverticulum is feasible and safe. It is acceptable alternative to the open technique implying thoracotomy. Surgeons performed such operations must be experienced in thoracic surgery and should be on alert regarding possible complications. Long-term results (after three months) is satisfactory too.
