OBJECTIVE

To assess the impact of Helicobacter pylori (HP) and duodenogastric reflux (DGR) on the development and treatment outcomes of hyperplastic gastric polyps (HGP).

MATERIAL AND METHODS

From 2017 to 2023, 352 patients with HGP (73.8% women) were examined, patients’ average age was 58±7.4 years. All underwent esophagogastroduodenoscopy with narrow band imaging and biopsy, HP testing via the ¹³C-urea breath test (¹³C-UBT).

RESULTS

318 (90%) patients had HGPs in the antrum, and 34 (10%) in the gastric body. Polyps measuring 1—4 mm were found in 214 (60.7%) patients, 5—9 mm in 131 (37.3%), and 10—14 mm in 7 (2%). According to the Paris classification, the polyps were distributed as follows: Ip in 73 (20.7%) patients, Is in 137 (38.9%), Isp in 85 (24.2%), and IIa in 57 (16.2%) patients. Chronic atrophic gastritis was detected in 298 (84.7%) patients, DGR in 227 (64.5%), and HP-associated gastritis in 198 (56.3%). Among the 287 patients who underwent ¹³C-UBT, 183 (63.8%) tested positive, in 47 (13.4%) tested negative morphology identified coccoid bacteria. HP infection was associated with the development of Is type HGP (P=0.01), and DGR was linked to low HP colonization (P=0.028). Eradication therapy led to HGP elimination in 208 (59.1%) patients, while 128 (36.4%) showed no change. Endoscopic polypectomy was performed in 104 patients.

CONCLUSION

HP infection is associated with an increased risk of HGP development, while DGR correlates with low HP colonization. False-negative ¹³C-UBT results were linked to presence of coccoid bacteria. Eradication therapy successfully eliminated led HGP in 59.1% of cases.
