Aim. To analyze the incidence of colorectal cancer (CRC), its dynamics in patients younger 50 years old in the Yaroslavl Region and estimate the prospects for implementation of a regional screening program for CRC considering the age structure of patients and condition of endoscopic service in the region. Material and methods. The incidence of CRC among the residents of the Yaroslavl region for the period 2013—2017 was analyzed considering Cancer Register 6FB data. Results. The incidence of CRC is being increased in the Yaroslavl Region (by 6.1% over 5-year period). Moreover, this value increased by 56% among patients younger 50 years old. The proportion of patients with early stages of colon cancer is being raised. The prevalence of CRC in the Yaroslavl Region is higher than in the Russian Federation. However, early diagnosis rate is also higher than in the Russian Federation as a whole. There are 1221 diagnostic colonoscopies and 79 polypectomies per 100 thousand people annually in the Yaroslavl Region. In the Russian Federation, these values are 659 and 68, respectively. A significant number of diagnostic colonoscopies affect both high detection rate of colon cancer in the region and early diagnosis rate. Conclusion. Positive dynamics with early diagnosis of CRC is associated with the condition of endoscopic service in the region. Introduction of CRC screening programs with fecal occult blood testing followed by colonoscopy in the case of the positive test should facilitate diagnosis at the early stages of cancer.
