OBJECTIVE

To assess safety of passive verticalization at intensive care unit.

MATERIAL AND METHODS

The study enrolled 24 patients including 19 ones with concomitant trauma and 5 patients with severe isolated brain damage. Each patient underwent 1 — 8 sessions of passive verticalization assisted by tilt table on the 8th, 5th and 11th days of ICU-stay. A total of 79 procedures were performed. During verticalization, 17 patients were on extended mechanical ventilation through tracheostomy tubes (51 procedures). Achievement of gravity gradient and development of complications were assessed. Each procedure was accompanied by arterial acid-base state analysis.

RESULTS

Gravitational gradient was achieved during 63 (79.7%) verticalization procedures. We had to interrupt other 16 (20.3%) procedures due to development of persistent orthostatic complications. Event-free verticalization took place with equal incidence in patients on mechanical ventilation and those with spontaneous breathing (52.9 and 53.6%, respectively). Ratio of non-severe-to-severe complications was 2:1 in patients on mechanical ventilation, in patients with spontaneous breathing — 1:2. Arterial acid-base state analysis showed improved oxygenation after verticalization sessions.

CONCLUSION

Passive verticalization in ICU is a safe method of early rehabilitation requiring continuous monitoring of patient’s condition and strict adherence to the protocol.
