Abstract:

OBGECTIVE

Identification of risk factors of obstetric cervical traumatism at term labor complicated by premature rupture of fetal membranes (PRFM).

MATERIAL AND METHODS

A retrospective cohort comparative study was conducted with 215 patients who had term labor complicated by PRFM. The 1st group included 49 patients with preterm delivery complicated by cervical rupture of various degrees, the 2nd group included 166 patients with preterm delivery and without maternal traumatism. Anamnesis data, peculiarities of the course of pregnancy and labor, perinatal outcomes were studied. Statistical analysis was performed using Excel MS and Statica 7.0 programs.

RESULTS

The predominance of nulliparios women in group 1 was found: 57.1% versus 40.4% in group 2 (χ2=4.32; p=0.04; OR 1.97; 95% CI 1.03—3.76), while pregnancy in group 1 was 1.8 times more likely to be anemic — 51.0% (χ2=8.76; p=0.004; OR 2.64; 95% CI 1.37—5.07). The risk factors for cervical rupture in PPFM and preterm gestation were found to be associated with a lack of biological readiness of the birth canal («immature» and «under-mature» cervix — 75.5%), the development of anomalies of labor (28.6%), and the use of operative delivery during labor (vacuum extraction of the fetus, episiotomy — 32.4%). The negative impact of obstetric traumatism of the mother during term labor complicated by PRFM on the morbidity of newborns was proved: the frequency of cephalohematoma increased 5-fold in group 1 and cervical trauma 5.7-fold.

CONCLUSION

The results obtained indicate the need for further research in this area to find possible methods for predicting birth trauma and improving obstetric and perinatal outcomes.
