The article deals with the problem of polypoid rhinosinusitis (PRS) in children. It gives modern views on the epidemiology, etiopathogenesis, and treatment of PRS in children. 83 (78.3%) children with PRS were diagnosed with allergic rhinitis, 22 (20.8%) cystic fibrosis, 4 (3.8%) ciliary dyskinesia (including Kartagener’s syndrome), and 34 (32.1%) atopic asthma. The use of current intranasal glucocorticosteroids, mometasone furoate (MF) spray in particular, is most promising in the therapy of children with PRS. It was shown that after 3 months of therapy with MF at a dosage of 100 µg/day in children less than 12 years of age and 200 µg/day (100 µg twice daily) in those aged 12 years and older, difficult nasal breathing was relieved in 68.4% of cases. At this stage of treatment, a decision was made to refuse surgical treatment and to continue MF therapy in these children under the dynamic guidance by an ENT specialist and an allergologist. The patients, in whom the effect of MF spray had been inadequate, underwent a planned surgical procedure. In view of the frequent association of PRS with allergic rhinitis, the use of intranasal glucocorticosteroids for children is a promising treatment for nasal polyposis in this category of patients.
