Abstract:

RELEVANCE

Infertility associated with a decreased ovarian reserve is one of the main unresolved problems of modern reproductive medicine. Due to the low adherence of patients to the use of oocyte donation as part of the IVF program, it is necessary to create an up-to-date concept for optimizing methods of infertility treatment in patients with extremely low ovarian reserve.

OBJECTIVE OF THE STUDY

Improving the effectiveness of infertility treatment in patients with reduced ovarian reserve using new surgical technologies.

MATERIAL AND METHODS

A comprehensive treatment of infertility was performed in 89 patients of reproductive age with a reduced ovarian reserve, including the stages of outpatient counseling, preoperative preparation of the examined patients, surgical treatment and postoperative management with ovarian function stimulation in the presence of conditions and indications. All patients included in the study underwent a one-stage surgical method for activating of ovarian function during surgery according to the method created by Academician of the Russian Academy of Sciences, L.V. Adamyan.

RESULTS

It was developed own scientifically based concept of managing patients suffering from infertility associated with a diminished ovarian reserve, which turned out to be effective in terms of positive dynamics of clinical and laboratory parameters, echographic criteria for ovarian reserve, optimization of ovarian function stimulation, as well as achieving pregnancy using own oocytes in 16 patients with birth to date 9 healthy children.
