OBJECTIVE

To analyze the effectiveness of various surgical corrections of false distal aneurysms after aorto-femoral bypass grafting.

MATERIAL AND METHODS

There were 21 interventions for false aneurysms of distal anastomoses of aorto-femoral bypass grafts between 2015 and 2020. Patients were divided into 2 groups depending on extent of surgical trauma: low-traumatic surgical techniques (13 (61.9%) patients) and conventional surgical interventions (8 (38.1%) patients).

RESULTS

We estimated the «don’t touch» technique in the treatment of false aneurysms of distal anastomoses of aorto-femoral bypass grafts. Its advantages over generally accepted standard interventions are proven. Histological analysis of arterial wall was performed after redo surgeries for false aneurysms of distal anastomoses of aorto-femoral bypass grafts. The immediate and long-term results of various surgical interventions were assessed. We analyzed the incidence of thrombosis, restenosis and recurrence of false aneurysms in postoperative period.

CONCLUSION

Temporary intraluminal balloon occlusion using the “don’t touch” technique minimizes external trauma to vascular wall. Shunt-plasty may be preferable for surgical correction of false aneurysms of distal anastomoses of aorto-femoral bypass grafts.
