Abstract:

OBJECTIVE

To describe the immediate and long-term results of arterial switch surgery in patients with dextro-transposition of the great arteries (d-TGA).

MATERIAL AND METHODS

We retrospectively analyzed clinical outcomes in patients with d-TGA who underwent arterial switch surgery between 2006 and 2021. There were 98 patients divided into 3 groups: group 1 — 54 (55.1%) patients with simple TGA, group 2 — 35 (3.,7%) patients with TGA and ventricular septal defect, group 3 — 9 (9.2%) children with TGA and comorbidities. Median age of patients was 11 days (range 1 day — 20 months). The mean follow-up period was 10 years. Demographic, pre-, intra- and postoperative data, as well as long-term survival and echocardiography data were assessed after 1, 5 and 10 years.

RESULTS

In-hospital mortality was 6.1%. The main cause of in-hospital mortality was acute coronary insufficiency. There was no long-term morality. Mild-to-moderate insufficiency of neoaortic and neopulmonary valves was observed throughout the follow-up period. Severe stenosis of pulmonary artery valve with indications for surgical correction occurred in 6 (6.1%) patients.

CONCLUSION

Nowadays, arterial switch surgery is a “gold standard” of radical correction of d-TGA with satisfactory early and long-term results. Nevertheless, all patients after these procedures need lifelong follow-up and regular echocardiography.
