OBJECTIVE

To develop an effective strategy for extracorporeal hemocorrection after complicated aortic surgery.

MATERIAL AND METHODS

We observed 206 patients aged 13-82 who underwent aortic surgery in 2000-2009 (group 1 — 102 patients) and 2010-2019 (group 2 — 104 patients) and had perioperative complications. We assessed incidence of complications and effectiveness of their management by using of extracorporeal methods. We used high-volume plasmapheresis, various methods of renal replacement therapy and extracorporeal treatment of sepsis.

RESULTS

In the 1st group, the complications associated with blood loss, intravascular hemolysis and long-term extracorporeal perfusion prevailed. In the second group, reperfusion syndrome was predominant. Moreover, the number of septic complications was increased. Early plasmapheresis over 70% of circulating plasma volume within 4-6 hours after surgery was effective for prevention of multiple organ failure in 70-80% of cases. Renal replacement therapy was required in 50 patients (24.3% of cases). Indications for plasmapheresis and renal replacement therapy were determined.

CONCLUSION

High-volume plasmapheresis (≥70% of circulating plasma volume) is advisable for prevention of multiple organ failure syndrome after complicated aortic surgery. Extracorporeal therapy of sepsis in these patients requires further researches.
