Aim — to evaluate the role of coronary angiography (CAG) before non-cardiac surgery to predict perioperative complications. Material and methods. The study included 150 patients who underwent CAG before elective non-cardiac interventions. Additionally, survey protocol consisted of cardiologist’s consultation, blood cell count, ECG at rest, 24-hour ECG monitoring, echocardiography and ergospirometry. Intraoperative and 30-day postoperative cardiac complications included death from cardiovascular cause, nonfatal myocardial infarction and stroke. Results. Perioperative complications developed in 20 patients: nonfatal myocardial infarction in 16 (10.7%), 4 (2.7%) died from cardiovascular cause. In patients with perioperative cardiovascular complications subtotal stenoses of the coronary arteries (25% vs. 6.5%; p<0.001, OR 4.5; 95% CI 1.33—15.15) and triple-vessel coronary lesion (25% vs. 6.2%; p<0.001; OR 5.1; 95% CI 1.47—17.56) were revealed more frequently. In multivariate analysis CAG data increased prognostic accuracy of preoperative examination for advanced abdominal surgery, interventions for oncological abdominal and urological pathology, as well as in patients with post-infarction cardiosclerosis. Conclusion. Subtotal stenosis and triple-vessel coronary lesion are associated with perioperative cardiac complications in various non-cardiac interventions.
