Venous adventitial cystic disease (ACD) is rare. About 50 cases are described in the world literature. The authors describe surgical treatment of venous ACD in a patient with symptoms mimicking deep vein thrombosis of the lower extremities. ACD of the common femoral and external iliac veins was diagnosed considering history, data of physical examination, ultrasound of lower limb vessels, CT angiography, magnetic resonance imaging, intraoperative findings and histological results. Resection of external iliac and common femoral veins was performed along anterior and medial walls for 4 cm. Wall defect was closed with a single-row suture using atraumatic needle. Vein diameter de novo was 4—5 mm. The patient was discharged in satisfactory condition. In long-term period, regression of edematous syndrome was observed under compensatory ectasia of the iliac vein. This clinical case can be considered as a variant of surgical treatment of venous compression syndrome induced by vascular wall neoplasms.

CONCLUSION

Venous ACD should be considered in differential diagnosis of acute venous thrombosis, post-thrombotic disease and deep vein venous malformation of the lower extremities.
