Abstract:

Endovascular coil embolization of the gonadal veins (EEGV) in patients with pelvic congestion syndrome (PCS) may induce deterioration of pelvic pain and tenderness along the embolized vein due to the development of post-embolization syndrome (PES).

OBJECTIVE

To analyze the incidence of PES in patients with PCS after EEGV.

MATERIAL AND METHODS

EEGV was performed in 67 patients with PCS. Steel coils 0.035’’ with a diameter of 8-12 mm and length of 10-20 cm were used for embolization. EEGV was left-sided in 58 cases, right-sided in 3 cases and bilateral in 6 patients.

RESULTS

PES included deterioration of pelvic pain, tenderness along the embolized vein and hyperthermia up to 37.5-37.8°C and was diagnosed in 13 (19.4%) patients. Non-steroidal anti-inflammatory drugs (NSAIDs) and venoactive drugs (VADs) were used to manage PES. The duration of NSAIds therapy ranged from 3 to 7 days (mean 4.2±1.1 days). VADs treatment duration was 2 months. PES symptoms were alleviated in all patients within 14 days and completely resolved by the 30th day after EEGV. Parametric vein thrombosis was revealed in 8 (14.8%) out of 54 patients without PES and in 2 (15.3%) out of 13 patients with PES.

CONCLUSION

Deterioration of pelvic pain, tenderness along the embolized vein and hyperthermia should be considered as manifestations of PES in patients who underwent EEGV. Duration of this syndrome does not exceed 14 days, and non-steroidal anti-inflammatory and venoactive drugs are effective for symptom relief.
