The objective of the present study was to evaluate the results of the combined surgical treatment of the patients presenting with trophic venous ulcers of the lower extremities. Material and methods. The study included 18 patients with trophic venous ulcers of the lower extremities associated with chronic venous disease (10 or 55% men and 8 or 45% women). The mean age of the patients was 51,6±10,6 years. The main causes underlying ulcer formation were chronic venous disease and post-thrombotic disease at the ratio of 1:1. Six patients underwent the autodermoplastic closure of the ulcers without the surgical correction of the venous blood flow. In seven cases, phlebectomy constituted the first stage of the treatment. The remaining five patients underwent simultaneous autodermoplasty and phlebectomy. Results. None of the patients developed postoperative complications. Engraftment of the cutaneous skin flaps occurred in all the treated patients. The trophic ulcer recurred in three (17%) of them. Conclusion. The combined treatment of trophic venous ulcers of the lower extremities provides a basis for their successful healing. The surgical strategy for the management of the patients presenting with persistent non-healing trophic ulcers should include such method as the operative intervention on the venous system and autodermoplasty.
