Acute ascending thrombophlebitis (AATP) is the most widespread emergency surgical pathology of the vascular system. The spread of thrombosis from superficial to deep veins associated with this pathology is a health- and life-threatening condition. There are two essentially different approaches to the solution of this problem, conservative and surgical. Despite the fact that the interest of surgeons in the AATP problem has a more than 100 year history, there is up to now no consensus as regards the optimal strategy for the prevention of deep vein thrombosis and pulmonary embolism in patients presenting with AATP. The objective analysis of the literature data gives ground for the conclusion that surgical prophylaxis of deep vein thrombosis and pulmonary embolism in patients with acute ascending thrombophlebitis is not given proper attention. The same is true of the reliable and safe approach to the treatment of such patients. The literature data available thus far suggest at least equal efficacy of conservative and surgical methods for the treatment of AATP.
