Abstract:

Background: Destructive pancreatitis is severe surgical condition with increased lethality and high rate of complications. Materials and methods: Analysis of treatment of 210 patients with mild and severe acute destructive pancreatitis is presented. Control group included patients with standard therapy while as patients of the main group (n-156) additionally underwent endoscopic major pancreatic duct stenting and were administered with high Octreotide doses to achieve intraductal pressure decrease, prevention and treatment of internal pancreatic fistula. Results: In the main group in contrast with control group more rapid reduction of serum amylase, lipase and leukocytal intoxication index were noted. Infection developed in 11% of the main group and 20% of the control group. 3% in the main group and 9% of patients in the control group died.
