Airway pressure release ventilation (APRV) differs from the conventional ventilation modes due to a significant inversion of the inspiratory to expiratory ratio. This mode is recommended for acute respiratory distress syndrome if the «traditional» approach is not effective. In recent years, ventilation strategy with APRV has significantly changed based and new physiological approach was justified. Various modes of APRV, respiratory physiology during ventilation in this mode, its effect on ventilator-induced lung injury and current evidence on the use of APRV for ARDS are reported in the article.
