Writer’s cramp (WC) is a task-specific dystonia, which is characterized by a selective disorder of motor skills of the hand, which makes writing much more difficult or impossible. Eighty six patients with idiopathic writer’s cramp (26 men, 60 women), aged from 12—66 years (middle age 39.6±12.5 y) were studied from 1995 to 2012. The age of onset was 31.3 years, and in 53.7% patients, the onset of WC was at the age of 30 years. WC refers to poorly treatable diseases. Conservative therapy of WC has a mainly empirical approach, which is ineffective. In WC therapy, one should follow a scheme of therapeutic effects, including neuro-rehabilitation (sensory-motor retraining) with the formation of a new motor program, an impact on peripheral factors, pharmacotherapy, psychological adaptation and socialization, to prevent WC in high-risk groups. Clonazepam is the basic drug in the treatment of WC. The use of combination therapy of WC allows the reduction of the dosage of clonazepam to 1.5—2 mg per day to prevent the progression of the disease. The duration of therapy is at least 1 year. The response to therapy with different degrees of expression was obtained in 79.3% of patients.
