Objective: to study immune homeostasis in premenopausal patients with vulvar lichen sclerosus in relation to topical corticosteroid use. Subjects and methods. Examinations were made in 93 patients aged 41 to 52 years with vulvar lichen sclerosus (a study group); 30 patients having no signs of vulvar diseases or recurrent chronic diseases at the examination formed a control group. According to the used topical agent, all the patients were divided into 3 subgroups: 1) 20 patients received topical hydrocortisone ointment; 2) 21 applied clobetasol ointment; 3) patients had the latter in combination with drug mixture-containing cream. The examinations included clinical, laboratory, immunological, cytological, histological studies, and plain and extended vulvoscopy. Results. Before treatment there was a pretreatment deviation from the normal value of immunocompetent cells in the patients of the study group. After treatment, the patients in Subgroups 2 and 3 displayed significant increases in both the absolute count and percentage of the major lymphocyte subpopulations: CD3, CD4, CD8, and CD16. The entire spectrum of expression of the receptors of neutrophils, monocytes, and lymphocytes, their impaired ratios, and decreased activity of their reception by vulvar tissues are of importance in immune homeostatic disorders in vulvar lichen sclerosus. Conclusion. The rapidest immune normalization due to the topical use of 0.05% clobetasol propionate in combination with drug mixture-containing cream has been established in premenopausal patients with vulvar lichen sclerosus, enabling the body’s load to be reduced by exogenous corticosteroids.
