As the analysis of publications shows, women with polycystic ovary syndrome are observed to have generalized hypersecretion of adrenocortical androgens, in basal state or in response to adrenocorticotropic hormone stimulation. The mechanism of these disorders remains unclarified and an analysis of candidate genes has revealed no specific genetic defects. The production of adrenal androgens in response to adrenocorticotropic hormone stimulation is closely related to the impaired mechanism regulating the peripheral metabolism of cortisol and to a lesser extent to the effects of extra-adrenal androgens, insulin resistance, and obesity.
