Abstract:

The purpose of this investigation was to study the specific features of changes in proangiogenic (placental growth factor (PlGF)) and antiangiogenic (soluble FMS-like thyrosine kinase-1 (sFlt-1) and endoglin (sEng)) in pregnant women with chronic arterial hypertension (CAH). The course of pregnancy, labor, delivery, postpartum period, and postnatal outcomes were clinically analyzed in 118 women. A study group comprised 69 pregnant women with Stage I CAH (without target organ involvement) (Grade I). A control group included 49 pregnant women having no severe extragenital diseases without an obstetric history and with current physiological pregnancy. The time course of changes in the serum levels of proangiogenic (PlGF) and antiangiogenic (sFlt-1, endoglin) factors was studied in all the pregnant women at 22-24, 28-34, and after 37 weeks gestation. The middle of the third trimester of pregnancy was marked by an altered balance between angiogenic factors in the sera of pregnant women with CAH manifesting as elevated antiangiogenic sFlt-1 and endoglin levels and decreased proangiogenic PlGF concentrations as compared to those in the control group. There was a compensatory increase in PlGF elaboration after 34 weeks of pregnancy. The index K reflecting an imbalance between the anti- and proantigenic factors may be used to predict complications in pregnant women with CAH.
