Health literacy (HL) is the knowledge, motivation and skills needed for people to find, understand, apprise and apply health information. For more than 12 years, research has been conducted in Europe. A study of HL at the population level was performed in Russia for the first time.
OBJECTIVE
To determine the level of HL of the population and identify the association of HL with the demographic and socio-economic characteristics of the population in three regions of the Russian Federation.
MATERIALS AND METHODS
A cross-sectional population study was conducted on a random representative sample of the population aged 18 and older in the Novosibirsk region, the Republic of Karelia, and the Republic of Tatarstan. The personal interviews in households method with the adapted version of the international questionnaire HLS19-Q22-RU was used. Four levels of HL were distinguished: inadequate, problematic, sufficient, excellent. The methods of frequency and correlation analysis (Spearman’s rank correlation, Pearson correlation χ2), and linear regression were used.
RESULTS
Data from 5560 respondents (average age 48.0±17.9 years) answered at least 80.0% of the questions were analyzed. Among them, 3022 (53.4%) respondents had a sufficient level of HL, 434 (7.7%) excellent, and 1467 (25.9%) and 737 (13.0%) of respondents had a problematic and nadequate level of HL, respectively. The multifactor model of the linear regression showed that five social determinants of the general HL explain 22.0% of the AH variations in the total sample: financial deprivation are the strongest predictor of HL (β= –0.20), followed by social status (β=0.16).
CONCLUSION
In the studied population of the Russian Federation, 38.9% have an insufficient level of HL. The strongest predictor of HL is the socio-economic status of the population. Thus, a social gradient of HL was identified, which indicates the need to reduce social inequality concerning information on the health and development of targeted interventions for identified vulnerable groups of population.
