
Результаты открытых и эндоназальных доступов в хирургическом лечении распространенных злокачественных опухолей придаточных пазух носа и основания черепа

Резюме
Злокачественные новообразования придаточных пазух носа и основания черепа встречаются редко. На их долю приходится 3—5% злокачественных опухолей головы и шеи и менее 1% всех злокачественных новообразований. Основными методами хирургического лечения злокачественных новообразований пазух носа являются трансфациальные операции, однако такие вмешательства связаны с серьезными осложнениями: косметические дефекты и рубцы на лице, раневые инфекции, длительное время восстановления, что приводит к увеличению продолжительности госпитализации и задержке адъювантного лечения. За последние 20 лет эндоскопическая эндоназальная хирургия значительно продвинулась вперед, расширив показания к ее применению. Этот технический прогресс сделал данный метод лечения настоящей альтернативой трансфациальным подходам в хирургии злокачественных новообразований придаточных пазух носа и основания черепа. Многочисленные исследования показали, что результаты выживаемости пациентов с заболеваниями придаточных пазух носа при эндоскопическом эндоназальном доступе сопоставимы, а в некоторых случаях даже лучше, чем при открытом подходе. Однако, учитывая редкость этого заболевания, персонифицированный выбор тактики лечения остается приоритетным на сегодняшний день.
