OBJECTIVE

To improve the quality of treatment in patients with bladder damage in locally advanced colorectal cancer.

SUBJECTS AND METHODS

From 1994 to 2012, surgical treatment results were retrospectively analyzed in 165 patients with bladder damage in locally advanced colorectal cancer at the A.N. Ryzhykh National Research Medical Center of Coloproctology. The patients were divided into clinical groups according to the volume of intervention. Group 1 (n=85) underwent bladder resection within the seromuscular layer; Group 2 (n=29) had sectoral bladder resection; Group 3 (n=35) subtotal bladder resection; and Group 4 (n=16) had cystectomy.

RESULTS

The 5-year survival rate was 57.6% in Group 1, 41.4% in Group 2, 40% in Group3, and 31.2% in Group 4. The volume of bladder resection did not affect the risk of local relapses and distant metastases. A high-grade tumor and metastatic lymph nodes were characterized by an unfavorable prognosis; the 5-year survival rate for these risk factors was 8.9%.

CONCLUSION

Organ-sparing operations on the bladder in patients with locally advanced colorectal cancer, even with its massive involvement in the tumor process, are oncologically appropriate and functionally acceptable.
