Abstract:

OBJECTIVE

To assess the results of surgical treatment in patients with inflammatory myofibroblastic tumors (IMT) of the lung.

SUBJECTS AND METHODS

The paper presents the authors’ experience in treating 11 patients with IMT of the lung at the N.N. Blokhin National Medical Research Center of Oncology in 2000 to 2015. There were 3 (27.3%) male and 8 (72.7%) female patients. The patients’ age ranged from 2 to 49 years, with a mean age of 20.1 years. Ten patients were surgically treated using the following operations: atypical lung resection (n=4), precision tumor removal (n=2), lobectomy (n=2), and pneumonectomy (n=2). Of them, 3 patients with peripheral neoplasms underwent economical thoracoscopic lung resections. One patient with a central exophytic tumor had endoscopic electrodestruction of the neoplasm.

RESULTS

The overall 5-year survival rate was 87%. A local recurrence occurred in 2 patients, of whom one experienced it at 22 months after precision tumor resection using a thoracoscopic approach; the other was detected to have a recurrence massive tumor in the posterior mediastinum at 44 months following left pneumonectomy in combination with pericardial resection.

CONCLUSION

The complexity of establishing the true morphological diagnosis of an IMT and the tendency to its invasive growth require adherence to strict principles of radical surgery with an adequate volume of lung resection to minimize the risk of recurrence.
