Abstract:

Objective: to provide the experience with endoprosthetic replacement for the wrist and ankle joints in patients with bone tumors of the distal forearm and distal lower leg. Subjects and methods. In 2006 to 2016, the National Medical Radiology Research Center, Ministry of Health of Russia, performed surgical treatment via segmental bone resection with replacement of the adjacent joint in 17 patients, including 5 (29%) men and 12 (71%) women. Their mean age was 49 (range 17—68) years. According to the morphological type of a tumor, 9 (53%) and 4 (24%) patients were found to have giant cell tumor and osteosarcoma, respectively; there was chondrosarcoma (n=1), malignant fibrous histiocytoma (n=1), adamantinoma (n=1), and leiomyosarcoma (n=1). Nine (53%) and 8 (47%) patients had a tumor in the distal tibia and in the distal radius, respectively. Results. The mean Musculoskeletal Tumor Society Rating Scale (MSTS) score after surgery was 73%. Infectious complications were diagnosed in 12%. Tumor recurrence was detected only in one (6%) patient. Conclusion. High MSTS scores, low risk of infectious complications, and early social adaptation in patients with distal extremity tumors allow a conclusion that endoprosthetic replacement is the treatment of choice for this group of patients.
