Survival is one of the most significant indicators of treatment for disseminated rectal cancer (DRC). Combination treatment gives patients a chance to survive 5 years. Subjects and methods. The results of comprehensive examination and treatment in patients with Stage IV (T3—4N0—2М1а—1b) DRC who had been treated in the Lugansk Regional Clinical Oncology Center in 1991 to 2012 and in the Grozny Republican Oncology Dispensary were given. Results. Five-year survival after synchronous resection of distant metastases (M1a) and primary tumor was 36.89% in the study group (R0), 10.28% in Group 1 (R1), and 2.63% in Group 2 of comparison (R2). In disseminated DRC with synchronous distant metastases at 2 or more sites (М1b), the combined procedures could achieve overall 5-year survival in 15.6% of patients in the study group with R0 cytoreduction, 7.4% in Group 1; none survived 5 years in Group 2 (R2 cytoreduction). Conclusion. The survival rates in patients with metastases (M1a and М1b) undergoing RO resection suggest that such interventions are effective.
