Abstract:

Redo thoracoabdominal aortic surgery after previous infrarenal aortic replacement is associated with high risk of perioperative complications. Cross-clamping of the descending thoracic aorta in these surgeries carries a high risk of ischemic renal injury with renal failure and potential fatal outcomes. The authors describe open surgical treatment of type IV thoracoabdominal aortic aneurysm in a 56-year-old patient who had previously undergone aortofemoral bifurcation replacement for infrarenal aortic aneurysm rupture in 2010. In November 2016, the patient underwent thoracoabdominal aortic replacement with antegrade bypass of the left renal artery. Postoperative period was uneventful. The patient was discharged after 10 days. CT aortography after 2 years found no postoperative complications.
