Abstract:

OBJECTIVE

To evaluate the efficacy and safety of intravenous ibuprofen under patient-controlled morphine analgesia in early postoperative period after cardiac surgery.

MATERIAL AND METHODS

Two postoperative analgesia regimens were evaluated in this observational study. The 1st group included 20 patients who were received infusion of ibuprofen 800 mg twice. The 2nd group included 20 patients who were anesthetized by patient-controlled morphine analgesia. Intensity of pain during motor activity was assessed using a 100-mm visual analogue scale (VAS). Inductive spirometry was additional objective criterion of effectiveness of analgesia.

RESULTS

In the 1st group, pain syndrome decreased from 46.75 mm to 24.25 mm (p=0.002) by the end of ibuprofen infusion. This trend continued at all stages, and mean VAS scores did not exceed 25 mm. Since morphine infusion was started 2 hours after tracheal extubation in the 2nd group, pain syndrome significantly decreased only after 4 hours and differed from previous stages. By the end of the first day, pain syndrome did not exceed 20 mm in both groups. Analgesic effect of ibuprofen positively affected the function of external respiration with 1.8-fold increase of maximum inspiratory capacity compared to baseline values. In the 2nd group, high rate of adverse events was associated with morphine and depended on its dose. Daily consumption of morphine per a patient was 12.4 mg and 22.6 mg, respectively (p<0.001).

CONCLUSIONS

Good analgesic and significant opioid-sparing effects, as well as safety of intravenous form of ibuprofen were observed for postoperative anesthesia after cardiac surgery.
