Abstract:

Aim — to present treatment of aortic graft infection after thoracic aortic surgery through midline sternotomy. Material and methods. There were 387 patients with thoracic aortic disease who were operated through midline sternotomy at the Aortic Surgery Department from November 2012 to November 2017. Aortic graft infection (AGI) occurred in 5 (1,3%) cases. Infected graft wrapping by omental flap was applied in 3 patients, pectoralis major muscle — in 2 cases. Early and mid-term (21 month on the average) outcomes were followed-up. Results. AGI occurred in 24 days after primary surgery (range 8—49) and was associated with deep sternal wound infection in 4 out of 5 cases. There were 50 days (18—87 days) between primary surgery and mediastinum repair for AGI. Mean hospital-stay after redo surgery was 14 days (range 11—21). Surgical success was observed in all patients. Four patients (80%) were discharged after smooth postoperative period. In-hospital mortality was 20% (1 patients who underwent coverage by pedicled muscle flap died from multiple organ failure). Recurrent AGI in mid-term period (6 months after redo surgery) occurred in 1 patient after previous graft wrapping by muscular flap. This patient died from stroke (25 months after primary surgery). Overall mid-term mortality was 40% (n=2). Omental flap coverage was followed by 100% freedom from recurrent AGI and survival in all 3 patients. Conclusion. Graft wrapping by omental flap is effective to eliminate AGI with minimal invasiveness.
