Aim. To perform comparative retrospective analysis of immediate results in patients with coronary artery spasm (CAS), in whom intracoronary selective nitroglycerin (NTG) administration was performed during coronarography and intraoperatively. Material and methods. We performed 3452 operations for the period 2013—2014 in our institution. Coronary artery spasm was diagnosed in 60 (1.7%) cases. Coronary spasm was treated using two methods: NTG administration into coronary arteries (shunts) during coronarography in 22 (36.7%) cases after operation (group 1) and intraoperative administration of NTG into venous shunts and selectively into coronary arteries in 38 (63.3%) cases (group 2). The method was chosen individually in each case. Results. The data of this study demonstrate efficacy of intraoperative NTG administration into aortic root or coronary bypass grafts for CAS management. This method reduces the incidence of myocardial infarction, mortality and ICU- and hospital-stay.
