The method of mitral valve replacement with complete anterior leaflet translocation is described. We studied 64 patients operated for mitral valve insufficiency, in whom complete anterior leaflet translocation was performed. Anterior leaflet translocation leads to convergence of papillary muscles what facilitates left ventricular remodeling and makes it a physiological form of an ellipse. Formation of a single fibrous papillary bundle has a positive effect on blood expulsion into the aorta due to the release of left ventricular outflow tract from internal structures. Presented echocardiographic data are evidence of intracardiac hemodynamics normalization, decreasing linear and volumetric parameters of left ventricle as well as increasing contractility. Described technique may be used in patients with such disease especially in case of preoperatively decreased ejection fraction due to technical simplicity and obtained good results.
